After you have completed the form, please make sure to press the Complete and Send button at the bottom to automatically send us your information. The security and privacy of your personal data is one of our primary concerns and we have taken every precaution to protect it.

Background Information

Oral and maxillofacial surgery requires additional years of hospital-based surgical and anesthesia training after graduation from dental school. As an oral and maxillofacial surgeon, Dr. Lewis manages a wide variety of problems relating to the mouth, teeth, and facial regions. Dr. Lewis practices a full scope of oral and maxillofacial surgery with expertise ranging from dental implant surgery and wisdom tooth removal to corrective jaw surgery. This also includes techniques designed to rebuild bone structure with minimal surgical intervention and optimal patient comfort. Our office can also diagnose and treat facial pain, facial injuries, and fractures.

Our staff is trained in assisting with Intravenous (IV) sedation and outpatient general anesthesia in our state-of-the-art office setting. Patients are continuously monitored during and after surgery.

Surgical Staff

The surgical staff at Southwest Center for Facial and Oral Surgery are experienced, certified oral and maxillofacial surgical assistants, who assist in the administration of IV sedation and surgery. We also have a trained registered nurse (RN) to assist in surgery. All staff are informed administrative personnel, well versed in health and insurance policies, and are CPR certified.

Scheduling

We are open Monday through Friday, 8:00am to 5:00pm. If you have pain or an emergency situation, every attempt will be made to see you that day.

We try our best to stay on schedule to minimize your waiting. Due to the fact Dr. Lewis provides many types of dental services, various circumstances may lengthen the time allocated for a procedure. Emergency cases can also arise and cause delays. We appreciate your understanding and patience.

First Visit

Your initial appointment will consist of a consultation explaining your diagnosis and treatment options. Occasionally, surgery can be performed the same day as the consultation. However, a complex medical history or treatment plan may require an evaluation and a second appointment to provide treatment on another day.

Please assist us by providing the following information at the time of your consultation:

  • Your surgical referral slip and any x-rays if applicable.
  • A list of medications you are presently taking.
  • If you have medical or dental insurance, bring the necessary completed forms. This will save time and allow us to help you process any claims.

IMPORTANT: All patients under the age of 18 must be accompanied by a parent or guardian at the consultation visit.

A preoperative consultation and physical examination is mandatory for patients undergoing IV anesthesia for surgery. Please have nothing to eat or drink eight hours prior to your surgery. You will also need an adult to drive you home.

Please alert the office if you have a medical condition that may be of concern prior to surgery (i.e., diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are currently taking any medications (i.e., heart medications, aspirin, anticoagulant therapy, etc.).

X-Rays

If your dentist or physician has taken x-rays you may request that they forward them to our office. If there is not enough time, please pick them up and bring them to our office. If additional films are necessary they can be taken at our facility.

Insurance Information

To accomplish this we work hand-in-hand with you to maximize your insurance reimbursement for covered procedures. If you have any problems or questions, please ask our staff. Financial Information

For your convenience, we accept cash, check, Visa, and MasterCard. We also offer CareCredit, which offer 6-12 months with no interest on any amount over $200. See their website at www.carecredit.com or ask the receptionist for more details. We deliver the finest care at the most reasonable cost to our patients, therefore payment is due at the time service is rendered unless other arrangements have been made in advance. If you have questions regarding your account, please contact us at 601-249-0045. Many times, a simple telephone call can clear up any misunderstandings.

Please remember that you are fully responsible for all fees charged by this office regardless of your insurance coverage.

We will send you a monthly statement. Most insurance companies will respond within four to six weeks. Please call our office if your statement does not reflect payments made by your insurance company during that time period. Any remaining balance after your insurance has paid its portion is your responsibility. Your prompt remittance is appreciated. We can make arrangements for a monthly payment plan, but that must be implemented prior to any actual procedures.
